          牙周再生治療臨床系列技術的建立和應用

          項目簡介

          牙周炎是危害人類口腔健康的首要疾病。牙周炎造成的牙齒支持組織和結構進行性破壞是成人牙齒缺失、老掉牙的根本原因。牙周炎治療的難點和關鍵是讓失去的牙周組織獲得再生,然而臨床上缺乏行之有效的方法。16年來,在國科金等10余項課題支持下(累計經費超過500萬元),本項目瞄準國際前沿,探索生長因子控釋、生物材料仿生、干細胞移植促進臨床牙周再生治療的新方法,建立系列技術體系用于牙周病臨床治療,經國內56家口腔醫院臨床驗證,療效確切。

          一、生長因子是促進牙周再生的重要工具,但直接使用極易失活,需反復使用,且易產生毒副作用。本項目成功研制具有不同藥物釋放效能的緩、控釋系統,可以滿足不同組織再生需求;在生物材料的多孔結構中,引入生長因子載體,實現藥物載體與組織工程支架一體化;將不同控釋載體與支架材料交聯整合,實現兩種因子有序、可控釋放,從而建立雙因子協同控釋技術。J Control Release前任主編,2012年發表述評,詳細介紹我們的工作,并再版雙因子控釋的結果。為進一步提升生長因子控釋效果,課題組創新性地利用微球載體膜孔結構接枝溫敏開關,實現生長因子智能化控釋。以上研究結果,在J Control Release等重要期刊上,發表論文SCI論文16篇。利用課題組研制的生長因子系列載體,大大減少了生長因子臨床應用的劑量和濃度,節約了成本,提高了治療效果。

          二、研制牙周仿生生物材料,是實現牙周再生的另一重要途徑。為達到生物材料結構仿生,課題組將人體組織脫細胞處理后,獲取細胞外基質信息,數字化并導入3D打印系統,建立精密仿生技術,體外成功復制出具有牙槽骨-牙周膜-牙骨質等復雜結構的支架材料;將歸巢誘導因子載體,與支架材料交聯整合,完成仿生生物材料細胞募集性設計,封面故事在Adv Biosyst上發表;隨后,通過分子機制研究,回答了內源性干細胞體內遷移、募集的機理。以四篇專論形式,詳細闡述了內源性再生策略、技術和方法,豐富和完善了牙周內源性再生理論。國外學者認為我們在牙周再生方面“邁出關鍵的一步”;并指出誘導機體干細胞歸巢是一項具有廣闊應用前景的臨床再生技術。由于系列工作積累,受邀在材料學權威期刊Prog Polym Sci上,發表83頁長篇綜述,總結作為臨床醫生,課題組對“人體來源生物材料”的思考、探索和實踐。此領域,共發表SCI論文15篇,其中5篇發表于材料學權威期刊Biomaterials雜志。動物實驗和臨床應用中,證實項目組研制的細胞募集性生物材料,具有激活機體內源性再生系統,募集內源性干細胞,修復牙周組織缺損的效能。

          三、牙周干細胞治療為牙周缺損修復和晚期牙周炎治療帶來新希望,但如何推動實驗室獲得的研究成果快速臨床轉化是必需解決的關鍵問題。課題組歷時4年,完成世界首個干細胞治療牙周炎隨機對照臨床試驗,加速干細胞牙周再生治療臨床轉化進程;圍繞臨床轉化中的關鍵問題,如細胞來源、培養方法、移植設計和療效評價等進行深入研究,共發表SCI論文17篇,并制定出一整套《牙周干細胞治療》臨床標準;被FDA臨床試驗中心同類研究采納。世界牙科研究會主席高度評價課題組的貢獻,對于整個再生醫學都將產生影響。進一步項目組將依托國家口腔疾病臨床醫學研究中心,組織開展多中心、大規模臨床試驗。

          通過以上研究,課題組建立了生長因子控釋、仿生生物材料設計和干細胞移植系列牙周組織再生治療技術體系,發明了3項新技術,完善了1個新理論,制定出1個新的臨床標準。本項目共發表SCI論文48篇;總影響因子242;發表中文論58,授權國家授權專利5項;參編國內外教材4部。建立了結構合理、年富力強的牙周組織工程與再生團隊,2017年獲批“陜西省重點科技創新團隊”。舉辦國家級繼續教育學習班12期、省級學習班6期,在國內56家口腔醫院,療效確切,未發現不良反應,取得良好社會和經濟效益。
